VVM DSL SERVICE CONTRACT

 This contract (the “contract”) is binding upon any person (“customer”) desiring a consumer DSL circuit installation from VVM Internet Service, Ltd. (“VVM”) to a residence or business.

Now, therefore, the parties agree as follows:

Article I. Statement of Work

VVM will provide the services (“service”) which will be a consumer DSL circuit communications link to the internet.

Article II. Duration of Contract

This contract shall be effective upon the submission of the Contract by Customer and receipt by VVM (the “effective date”).  This contract shall continue in effect, unless sooner terminated as provided in this contract, for a period of one year after the effective date.

Article III. Estimated Cost and Payment

A.     Customer shall pay VVM for one month of service in advance along with any associated one-time costs.  Service activation cannot be scheduled without advance payment.  VVM will invoice customer for the one-time cost, or any other subsequent one-time charges.  VVM will invoice customer monthly for the monthly recurring charges.

Invoices shall be paid within ten days of receipt by customer.  If payment is not received within ten days the account may be suspended and a ten dollar reconnect fee must be paid prior to the account being reactivated.

C.     Any and all fees resulting from any collections activities for any past due amounts are the sole responsibility of the customer.

Article IV. Termination

A.     Customer agrees to pay VVM a minimum termination fee of $200 in the event this contract is terminated prior to twelve months.

B.     VVM reserves the right to terminate customers access for any violation of the VVM Terms and Conditions without refund.  Customer will also be responsible for a minimum contract termination fee of $200.

C.     A thirty day written notice is required for termination of service.  Customer is responsible for payment for service thirty days after the written termination notice is received.

Article V. Disclaimer of Warranties

VVM EXPRESSLY DISCLAIMS ALL WARRANTIES, EXPRESSED OR IMPLIED, RELATING TO THE SERVICE INCLUDING, WITHOUT LIMITATIONS, ITS ACCURACY, RELIABILITY, COMPREHENSIVENESS, COMMERCIAL VALUE, MERCHANTABILITY, FITNESS, OR SUITABILITY FOR ANY PARTICULAR PURPOSE.
